What information do we collect about you?

When visiting our website, we do not automatically collect any information about you. We provide several means of contact including telephone, email and postal address through which enquiries and appointments can be made.

When contacting us via telephone, calls to our mobile number (07792 305480) are answered directly by NHSC staff. Calls to our landline number (01933 224454) are answered on our behalf by the Oxford Street Therapy Centre building reception, where you will be given the opportunity to leave your contact information with the receptionist who will then deliver your message to the NHSC before erasing their record of it.

During an initial appointment to visit our clinic and meet with a Trichologist, you will be provided with a short questionnaire covering your contact information, history of medical conditions, medicines and treatments currently being received, alongside any allergies you may suffer from. You will be asked to give consent for these details to be entered into your case history, alongside information and observations regarding your hair and scalp type (including your racial background where pertinent to treatment), family history of hair loss or relevant medical conditions, and any hair or scalp conditions you may be suffering from. This consent forms our legal basis for handling your data under the conditions and obligations of GDPR.

With your additional consent, we offer to store a photographic history of your hair and scalp in your case history, in order to monitor and review the progress of conditions and the effects of treatments provided.

How will we use the information we collect about you?

The information you provide during an enquiry will be kept in an appointment record if you should request that we provide our services to you as a patient, otherwise enquiry messages and the information they contain will be deleted within 6 months.

At your first appointment, your case history file will be created. Your case history file is retained by the NHSC, kept in a physical record in a securely locked location for the duration of your treatment and for up to seven years afterwards for the purposes of providing any follow up treatments and for maintaining our tax records. In addition, information from your case history file may be entered onto our password protected computer systems for the purposes of providing you with letters of diagnosis or referral to your GP or dermatologist. These letters are always sent directly to the patient who may then personally consent to their use by a GP or other third party.

Your case history and all personal information you provide is used strictly within the NHSC for the provision of treatment and never shared or disclosed to a third party without your express consent, or in the unlikely event you are unable to provide consent toward protecting your vital interests.

Marketing

We do not engage in marketing using the information you supply to us.

Access to your information and correction

You have the right to request a copy of the information we hold about you. If you would like a copy of some or all of your personal information, please email or write to us at the address at the top of this page.

You also have the right to request that we correct or delete information held about you. If you would like to request that we alter our records, please email or write to us at the address at the top of this page.

Cookies

Cookies are text files placed on your computer to collect standard internet log information and visitor behaviour information. We use YouTube to embed video content in our website, clicking to play this will set a Google YouTube cookie.
